Celiachia e osteoporosi

La celiachia crea uno stato d’infiammazione intestinale che impedisce il corretto assorbimento delle sostanze nutritive contenute negli alimenti ingeriti e fra queste anche la vitamina D e il calcio, indispensabili per la salute delle ossa.

La diminuzione del tono calcico della massa ossea determina un’osteopenia che, col tempo, può sfociare in un’osteoporosi (nei soggetti celiaci la percentuale di casi varia dal 35 all' 85%)

Qualora la celiachia sia diagnosticata in età pediatrica è sufficiente il rispetto della dieta aglutinata per ristabilire un corretto apporto di calcio e vitamina D ed evitare così il sopraggiungere di complicazioni.
Uno studio condotto a Triste, presso il reparto di gastroenterologia dell'Istituto Burlo Garofolo, ha, infatti, dimostrato che tutte le alterazioni metaboliche e della mineralizzazione ossea riscontrate nel bambino celiaco al momento della diagnosi, si normalizzano nel giro di 6-12 mesi dall'inizio della dieta aglutinata.
Il Prof. Alessandro Ventura, Direttore della Clinica Pediatrica triestina, ha dichiarato “lo studio che abbiamo portato a termine nel nostro Istituto è il primo studio sistematico sul metabolismo del calcio in età infantile. Da esso abbiamo tratto un insegnamento fondamentale: è vero che la celiachia danneggia l’osso ma, soprattutto in età pediatrica, non serve monitorare l’evoluzione del danno a questo tessuto con esami specifici quali, per esempio, la DEXA. Molto più produttivo è concentrare la propria attenzione sul fatto che il bambino osservi con regolarità e attenzione una dieta priva di glutine; in questo il ruolo dei pediatri può rivelarsi determinante

Qualora, invece, la celiachia sia diagnosticata tardivamente in età adulta, lo squilibrio nell'assorbimento di calcio è generalmente tale che il semplice rispetto della dieta senza glutine non è sufficiente a far regredire il danno osseo.
In tali casi, pertanto, è indispensabile monitorare periodicamente il contenuto di calcio presente nelle ossa tramite degli esami specifici ( Densitometria Ossea), ed eventualmente intervenire con un complemento farmacologico di calcio e vitamina D.
